Download and complete this form and call your local RSVP to schedule a volunteer matching appointment with the RSVP county coordinator. RSVP is for volunteers fifty-five years and older:

Complete this form each time you want RSVP to recruit a volunteer for your agency. Print a copy of the request form for your files, “submit” at the bottom of the page and your complete request will come directly to RSVP. You will be contacted by an RSVP representative to confirm details.
Memorandum of Understanding (MOU)
Please download this agreement form and complete and return it to RSVP to begin your RSVP volunteer association. RSVP partners are required to be 501c3 non-profit or public organizations.
